[Cable Card] "Channel Not Authorized" on several HD and Premium Channels

We have spent several hours on the phone with both Comcast and TiVo technicians over the past two months regarding an ongoing issue of not being able to view premium channels (HBO, Showtime) and several HD channels using a Motorola cable card in our TiVo Premiere XL. We've now swapped out the cable card 3 times and still have the same issue. We've replaced all splitters to increase the signal strength (we were getting 45% figures on Tuner 1) and both tuner's are in the 85% range, with a ~30db for SNR. When trying to access some HD channels and premium channels (we also cannot access Xfinity OnDemand) we get either this light gray screen that says: "This channel is not authorized. Contact your cable provider for more information" It will also list out the cable card ID, SN, Data ID, etc. and provides a phone number for Comcast that you need to call and activate cable card. The other error messages we encounter are either "unable to locate signal" or "you do not currently subscribe to this channel". Please help! We've spent several frustrating hours over the past couple of months with both Comcast and TiVo and we'd really like to get this resolved. I'm tired of not being able to access channels we continue to pay a lot of money to view.
